Here is how the model will work-Your doctors will order tests and prescribe treatments while he/she and I follow your case together to help support your health and healing process. This model has worked well for other healthcare providers I worked with who did not have sufficient experience treating complex medical problems such as chronic Lyme disease/PTLDS, mold toxicity, or other chronic illnesses where multiple inflammatory factors on the 16-point MSIDS model are underlying your illness.

The excerpt below comes from ‘Why We Stay Sick’ ‘Ending Chronic Illness’ (Simon and Schuster, 2026). It is not the complete list found in the book, but provides a general guideline for basic nutritional support based on common health challenges we all presently face:

Level 1: Basic Nutritional Support

These provide basic support for inflammation, immune function, detoxification, and your microbiome. These are meant for everyone and address a majority of the inflammatory pathways discussed in ‘Why We Stay Sick’. One of the main lessons I learned while writing my new science book and doing a one year deep-dive into the medical literature, is that the majority of chronic illnesses now affecting over 50% of Americans (one in two people now have at least one chronic illness, and 25% have two or more) are caused by chronic inflammation.

The inflammation can come from any and all of the 16 factors on the MSIDS model described in my prior bestselling books ‘Why Can’t I Get Better?’ Solving the Mystery of Lyme and Chronic Disease’ (NY Times Bestseller, St Martin’s Press, 2017), ‘How Can I Get Better? An Action Plan for Treating Resistant Lyme and Chronic Disease (National Bestseller, St Martin’s Press, 2017). It is also described in detail in our publication in the journal Healthcare:

If you are not used to taking supplements, I suggest starting with the three most essential:

  • N-acetyl-cysteine (NAC) e.g., Xymogen
  • Alpha lipoic acid (ALA) e.g., Xymogen and
  • Glutathione (GSH) e.g., Orthomolecular (doses are listed below)

These help block a switch in the nucleus of our cells called NFKappaB, which is a major switch that turns on inflammation. This pathway is involved in multiple disease processes including COVID and Lyme disease.

Then after a few weeks when your body has adjusted to these supplements, add the next level of nutraceutical support with:

  • Curcumin, i.e., turmeric (Xymogen, Curcuplex) and
  • Sulforaphane, i.e., broccoli-seed extract (NuMedica, Brocollinate)

These stimulate important anti-inflammatory and detoxification pathways, called Nrf2.

Most people will tolerate these well, and then you can add:

  • an Omega 3 FA (fish oil) once a day (QD) such as OrthoOmega (Orthomolecular)
  • a Vitamin D supplement once a day, i.e., K-Force, K2D3 (Orthomolecular) and
  • a multimineral supplement, i.e., MinRx (Xymogen).

The last ones to add to this basic nutritional support protocol are immune support

  • -Immunotix (e.g., 3-6 Beta glucan (Xymogen)
  • -Vitamin C and
  • -a probiotic, which can be rotated (Theralac, Master Supplements; Orthobiotic, Orthomolecular; Probiomax daily, Xymogen).

Details of these nutraceuticals are listed below.

Key code: QD=once a day; BID=twice a day; TID=three times a day; HS=taken at bedtime

Level 1 Basic Nutritional Support

  • NAC (Xymogen) 600 mg BID (twice a day, usually after breakfast and dinner). This supports glutathione production, the master anti-oxidant in the body. Since it tends to thin the blood a bit, if you are on any aspirin or anti-coagulants, check with your doctor regarding its use. N-acetylcysteine (NAC) functions as a potent anticoagulant by reducing the size and activity of von Willebrand factor (VWF), a blood clotting factor, thereby interfering with platelet aggregation and the formation of blood clots. It also directly impacts the coagulation cascade, though research shows conflicting results on how significant this effect is. I have been on it for decades as have my patients, and no one has complained of excessive bleeding, but if I cut myself shaving, it does take longer to clot. This effect of NAC, independent of its effect in regenerating glutathione and acting as biofilm agent, may explain in part why we had excellent results using it in COVID-19 where microclots have been identified as part of the reason why some people have gotten ill. More research is needed on that subject. Although I do not explain in detail the potential benefits and side effects of each supplement below, I will give you a few examples.   • ALA *(Alamax, Xymogen) 600 mg QD (once a day) or BID (twice a day). The dose depends on your health risks, and sensitivity to hypoglycemic swings as rarely higher dose alpha-lipoic acid will increase blood sugar swings. Alpha lipoic acid is a powerful anti-oxidant that is both water soluble and fat soluble (it penetrates into many areas of the body), regenerates intracellular glutathione, supports mitochondrial production, is helpful in insulin resistance (e.g., metabolic syndrome, Polycystic Ovarian Syndrome [PCOS]) and even has benefits supporting liver function. Again, more information on each supplement can be found by searching for the supplement in the A-Z directory and clicking on the link. Always check with your healthcare provider if you have questions.
  • Glutathione (GSH), Orthomolecular Products, 500 mg BID. Higher levels, up to 2000 mg BID or TID (three times a day) can be used temporarily for up to 2 weeks for acute viral infections (Covid, flu, etc.), and Herxheimer reactions (killing off bacteria in Lyme disease). I generally take 500 mg twice a day for immune, inflammatory and detoxification support.
  • Glyteine (Continual G) 400 mg QD. This is a glutathione (GSH) precursor for those who have difficulty tolerating glutathione. A comprehensive protein diet with amino acids cysteine, glycine, and glutamic acid are also building blocks for this essential antioxidant.
  • Curcumin (Xymogen, Curcuplex) 500 mg BID. This is one of the 4 nutraceuticals that stimulates the Nrf2 pathway, involved in inflammation and detoxification.
  • Sulforaphane (Broccoli-seed extract, NuMedica) 30-100 mg BID. NuMedica makes a sulforaphane which is 60 mg, and it can be used once or twice a day depending on your requirements for antioxidant, anti-inflammatory, detoxification support. This is the second nutraceutical to support the Nrf2 pathway.
  • Mercury- free Omega 3 Fatty Acid (OrthoOmega)1-2 grams QD. Fish oils help support healthy prostaglandins, another essential inflammatory (or anti-inflammatory) pathway.
  • K2D3 5000 IU (vitamin D QD, i.e., K-Force, Orthomolecular). Vitamin D acts as a hormone for bone health and affects our immunity. Adding K2 gives you extra cardiovascular support.
  • Multimineral supplement like MinRx (Xymogen): the dose is two twice a day with meals, or 4 once a day. Four capsules contain 200 mg of magnesium malate and glycinate, plus copper (3 mg), zinc (30 mg), iodine (150 mcg) and trace minerals. Minerals should generally be taken away from certain medication (antibiotics, hormones) to avoid interactions. Speak to your healthcare provider about medication you may be taking that could interact. These minerals are required for essential detoxification reactions, hormone production, with effects on inflammation.
  • 3,6 Beta glucan (e.g., Immunotix, Xymogen) 500 mg QD. This is immune support which supports natural killer cells, needed for viral infections, cancer, etc. Again, please search for a product on our site with the A-Z directory or put the name in the search box, and more detailed information can be found on each supplement.
  • Vitamin C 1 gram QD. This is primarily for those who do not get adequate fruits and vegetables in their diet and have the need for extra anti-oxidant support.
  • Probiotics (e.g., Theralac, Master Supplements; Orthobiotic, Orthomolecular Products; Probiomax 30-100 billion, Xymogen) QD. The doses will depend on the amount of microbiome support you require based on prior uses of antibiotics, infections, etc. I take one a day of each and rotate them in my nutraceutical regimen.

Health Support for Different Medical Conditions:

I have included in the examples below, some of the nutritional support that can be customized to your individual health needs, whether it be blood sugar support, liver support, etc. For example, under ‘Sleep support’ you will find some of the different nutraceuticals we have found to be effective for insomnia (still requiring getting to the underlying source(s) of why you are not sleeping!) which oftentimes can be combined with medication to help avoid polypharmacy and potential side effects whenever possible. Again remember, even though I am describing the potential uses of these nutraceuticals to support your health, they are, as per the FDA, not meant to “diagnose, treat, cure or prevent any disease”. That being said, all recommendations below come from a deep-dive into the peer-reviewed medical literature regarding their use. My book will have approximately 2000 scientific references backing up our research and recommendations that you and your doctor can review for more detailed information.

Level 2: Advanced Detoxification/Liver Support

These nutraceuticals are for those who are actively detoxifying heavy metals, mold, and other environmental chemicals. They can be added to Level 1 support when living in crowded cities or near highways where toxic loads are higher, or when dealing with fatty liver (i.e., Non-alcoholic Fatty Liver disease, NAFLD) or other liver inflammation (hepatitis, etc.).

Add to Level 1 Supplements

  • Methylation support, i.e., Methyl protect one QD (B6, 10 mg; methyl B12, 1000 mcg; methyl folate, 3400 mcg; Xymogen). This is also helpful if you have elevated homocysteine levels (a cardiovascular risk factor)
  • Phosphatidylcholine 1 gram QD (2.5-3 grams BID for active mold detoxification). We use Orthomolecular’s BioPC Pro (powder) or Xymogen’s Phosphaline 4:1.
  • DIM (Diindolylmethane) 2 QD (150 mg, Xymogen) and Medcaps DPO one BID (extra phase I, Phase II liver support, Xymogen). DIM consists of two molecules of indol-3-carbinol found in broccoli and supports phase 1 liver detoxification pathways. Medcaps contains vitamins, herbs (milk thistle, etc.) that support both phase 1 and 2 liver detoxification. MedCaps also contains some NAC and ALA and B vitamins, so if you combine it with Level 1 Basic Nutritional Support, make sure you are not taking too much of any one supplement.
  • Glucomannan 1.6 grams (Optifiber Lean, Xymogen) one scoop QD if elevated trichothecenes, one hour away from medication/supplements
  • Clay/charcoal 1 QD for mold toxins (GI Detox, Bio-Botanical Research) 2 hours away from medication/supplements; may increase dose (2-4) based on tolerance, constipation, and levels of toxins.
  • Extra Mag++: magnesium malate 100 mg, 2-4 BID (NutraMedix). Magnesium is needed in over 300 detoxification reactions, and this form of magnesium also supports mitochondrial function, can help with constipation, while helping to bind aluminum.
  • Saccharomyces boulardii 1 QD (Orthomolecular) helps reduce mycotoxin production, improve GI health/detoxification, and improve immune system support (also crowds out Candida). In mold protocols, this is usually used up to three times a day.

Additional Liver Support
For NAFLD (Non-Alcoholic Fatty Liver Disease)/NASH (Non-Alcoholic Steatohepatitis)
Apart from weight loss, a healthy low carbohydrate diet, regular exercise, and avoiding alcohol:

  • Berberine up to 1.5 grams/d (CM Core, 3 a day, Orthomolecular), Vit E (Annatto E, Designs for Health) up to 800 IU/day x 1year or longer (monitor cardiovascular status)
  • Silymarin (milk thistle) 140-210 mg BID/TID (MedCaps DPO, Xymogen contains 131.25 mg of Milk Thistle Extract per serving), evaluate potential interactions with immunosuppressants/chemotherapy.

For Hepatitis
Apart from antiviral medication for hepatitis B/C, use the same recommendations as NAFLD, plus add branched chain amino acids with extra zinc. For Hepatitis B, TCM + medication entecavir reduced liver fibrosis in studies.

Level 3: Mitochondrial Support: Healthy Aging, Whole Body Health, and Renewed Energy

This is important if you have/recovered from an infectious illness or an environmental challenge like mold that adversely affects mitochondrial function due to increased free radical/oxidative stress. It would also be advisable to add Level 3 support if dealing with neurocognitive challenges, since mitochondrial dysfunction is one of the issues underlying neurodegenerative diseases like Alzheimer’s disease (AD) and Parkinson’s disease (PD), or if you suffer from a chronic fatiguing illness, like ME/CFS, FM, Long Covid, Chronic Lyme, etc.

Add to Level 1 Supplements (and Level 2 Supplements If Needed)
Normally these are added in post-treatment protocols after detoxing mold or treating chronic Lyme disease and co-infections. Those with cardiomyopathy and congestive heart failure might also benefit from mitochondrial support.

  • Glycosylated phospholipids TID (NT Factors, or ATP 360, Researched Nutritionals, 3 QD)
  • Acetyl-L-carnitine 500-1000 mg BID (Xymogen, carnitex)
  • CoQ10 (100-400 mg/d, Researched Nutritionals CoQ Power)
  • NADH 5 mg QD-BID (ENADA)
  • D-Ribose (RibosCardio, Researched Nutritionals) one scoop BID for 1-2 months (due to potential increases in glycosylation, use lower dosing if you have a history of elevated blood sugars).
  • L-ergothioneine 12.5 mg one a day (Mitoprime, Xymogen)
  • Nicotinamide riboside 300 mg QD (MitoNR, 2 QD, Designs for Health)
  • Urolithin A 500 mg BID (this is produced by your microbiome, by consuming foods rich in ellagic acid and ellagitannins like berries, nuts, and pomegranates. It is also made when you take prebiotic fibers. I eat organic berries and nuts every day and use Orthomolecular’s MGP-Fiber, one scoop per day to support a healthy microbiome, and naturally produce Urolithin A because I am already taking so many nutraceuticals to support my health!)
  • Cordyceps 1000-3000 mg QD (TCM, Dr Zhang)

Level 4: Gastrointestinal/Candida/Allergy/Mast Cell Support

Those dealing with gastrointestinal inflammation, such as colitis, leaky gut with food sensitivities, and/or mast cell activation with allergies/elevated levels of histamine should consider adding level 4 supplements. Always review all recommendation with your healthcare provider before ordering these supplements.

Add to Level 1 supplements. Those with unbridled inflammation can add Levels 2 and 3 if needed.

For Leaky Gut

  • Bovine immunoglobulins, like SBI Protect (Orthomolecular) or MegaIgG 2 capsules BID (Microbiome Labs) with Megaspore BID day with Bacillus species (Microbiome Labs)
  • Extra microbiome support with a prebiotic (Trufiber, one tsp QD, Master Supplements; Fiber MGP, Orthomoleuclar, one scoop QD).

For Mast Cell Activation

  • DAO (diamine oxidase), e.g., HistDAO (Xymogen) 1-3 capsules before meals, to degrade histamine
  • PEA 800 mg/luteolin 267 mg (Sunbalance, Master Supplements, 3 QD)
  • Quercetin 500 mg BID (HistaQuel, Researched Nutritionals, two BID also contains perilla seed extract, black cumin extract (Nigella sativa) and stinging nettle extract)
  • Vitamin C 1 gram BID.

To Block Leukotrienes

  • Black cumin (Nigella sativa) 1-2 grams QD: This herb is present in Histaquel 2 BID (Researched Nutritionals)
  • Curcumin 500 mg BID (Xymogen, Curcuplex)
  • Coptis root (TCM) 100 µg/mL QD (TCM, Dr Zhang)
  • Omega 3 FAs 1-2 grams QD (OrthoOmega, Orthomolecular Products)
  • Quercetin 500 mg BID or higher (this is also found in Histaquel, Researched Nutritionals)
  • Resveratrol 150-1000 mg QD (Resveratrin, Xymogen)

For Candida Overgrowth

  • Probiotics with Lactobacillus, Bifidobacterium, (Theralac, Tru-Bifido [Master Supplements] -Orthobiotic (Orthomolecular), Probiomax 30-100 billion(Xymogen) + saccharomyces boulardii QD (Orthomolecular)
  • Candicid Forte (Orthomolecular): up to 2 TID; contains caprylic acid along with berberine HCl, Pau d’Arco, oregano, rosemary, cinnamon and ginger
  • Candibactin-BR: (Metagenics): CandiBactin-BR® combines berberine hydrochloride –(Berberine 500-1500 mg QD is suggested for Candida support) along with Oregon grape extract, and coptis root extract, with a proprietary extract of Chinese herbs—including ginger, licorice, and skullcap—to support detoxification of the liver and gallbladder, as well as healthy immune, detoxification, and elimination functions
  • Oregano oil 300-1000 mg/d (divided doses). Oil of Oregano by Designs for Health, starting one a day. Each softgel provides 36 mg of carvacrol and thymol from an impressive 60% to 75% carvacrol oregano oil. Higher doses (150 mg per two capsules) are found in Doctor Inspired Formulations cinnamon/clove/oregano oil capsules: usual dose is one twice a day for biofilm, Candida support.
  • Lauric acid (Lauricidin, coconut oil) ½ scoop/d, slowly increasing
  • Garlic (Allicin, TCM): one capsule HS, slowly increase to one TID

Level 5: Hormonal Support

Those with evidence of low adrenal function, sex hormones, thyroid function, or those in high- stress environments needing extra hormonal support.

Add to Level 1 Supplements (and Levels 2, 3, 4 as Needed)

For Adrenal Support

  • Adapten-All (Orthomolecular) 2 QD or Adrenal Essence (Xymogen) 2 QD. These are adaptogenic herbs that balance adrenal function, and contain ginseng, rhodiola (150 mg), schizandra berry (150 mg), ashwagandha (75 mg), licorice root (75 mg), pantothenic acid (350 mg) with B vitamins, C (200 mg) and E (13.4 mg). Whenever adding a supplement with vitamins and herbs, check for potential drug/nutrient interactions.
  • Adrenal glandulars (bovine sourced) like AdrenaLiv (Xymogen) or Adrenal Complex (PHP Products) are taken at a dose of 1-4 a day depending on cortisol levels. These come from countries known to be free of prion disease. They should not be used in those with alpha gal allergy, i.e., AGS (and gelatin capsules should also be avoided in those with AGS).
  • DHEA (Dehydroepiandrosterone) 5 mg QD or more (Metagenics, 1 spray sublingual, hold 30 seconds and swallow) is added for extra stress and low serum levels as it has been shown to have positive effects on bone density, cardiovascular function, sexual health, weight management, hormonal balance, and mental health. Hormone levels need to be followed using adrenal/sex hormone precursors like DHEA and pregnenolone.
  • Pregnenolone (Orthomolecular) 10 mg, 1-4/day (a precursor of adrenal and sex hormones). In some with low sex hormones, there is a process called a ‘pregnenolone steal’ where your body shifts the production of sex hormones to adrenal hormones. This can be helpful in those circumstances apart from providing other adrenal support.

For Thyroid Support

  • Selenium 200 mcg QD (selenium is part of the multimineral supplement MinRx, 4 a day from Xymogen)
  • Iodine 200 mcg QD (some iodine is in MinRx 4 a day, Xymogen)
  • T-150 glandular (Xymogen) 150 mg QD or more, depending on symptoms and thyroid function. Any time a glandular product is used, follow-up hormone testing is recommended.

For Sex Hormone Support

  • DHEA (as above, converts into testosterone in men and women; do not mix with testosterone)
  • Pregnenolone (as above)
  • Testoplex Plus 2 QD (Xymogen) which contains PrimaVie Shilajit (500 mg) and LJ100 -Eurycoma longifolia extract (200 mg) to support healthy testosterone levels. This helps to bump testosterone off protein binding sites *(SHBG) to increase free, active levels of the hormone (free testosterone).
  • L-Arginine, mentioned below, 2-6 grams/day to support hormone production and increase blood flow. Usually L-Arginine is used for cardiovascular support and nitric oxide production. Be careful mixing with medication like nitrates or drugs like sildenafil because of potential lowering of blood pressure. See the write-up below for more potential interactions.

For Glucose/Insulin Support

  • Soluble fiber slows down the absorption of simple carbohydrates and is a natural GLP-1 agonist (like semaglutide), with studies showing it may also improve insulin sensitivity and satiety.
  • Fiber MGP (Orthomolecular) one scoop QD is my favorite. Foods rich in soluble fiber include oat bran, beans, and legumes (lentils, chickpeas, kidney, and black beans), flaxseed and chia seed, almonds, walnuts. Slowly introduce fiber-rich foods to minimize digestive discomfort with increased hydration to avoid constipation. A Mediterranean diet is best for overall health.
  • Cordyceps 1000-3000 mg QD (Dr Zhang, TCM)
  • Cinnamon 1-6 grams/d, but watch for MCAS (we used cinnamon/clove/oregano oil from Doctor Inspired Formulations as our main source)
  • Berberine 500-1500 mg QD (CM Core, Orthomolecular, 3 a day contains 1000 mg and 200 mg of alpha lipoic acid
  • Benfotiamine, up to 300 mg BID (Xymogen, Benfotiamine, 2 a day)

Level 6: Cardio-Vascular Support

Add to Level 1 and 3 Supports as Needed

For Increased Nitric Oxide Support
Important for those with heart disease, hypertension, erectile dysfunction [ED], or wanting to increase athletic performance).

  • L-Arginine 2-6 grams QD, also found in supplements containing beet root extracts, hawthorn berry extract, L-citrulline like CircO2 (Advanced Bionutritionals). Those with allergies, asthma, migraines, and herpes reactivation should use L-ARG with caution. Also, it may interact with anti-coagulants, blood pressure, and cardiac medicine (nitrates), diabetes medication, potassium sparing diuretics and erectile function medication (sildenafil, tadalafil) lowering blood pressure, blood sugar, and increasing bleeding. Anyone on these medications should check with their HCP.

For Endothelial Support (when the inner part of blood vessels isn’t working properly)
In diseases including Long Covid, Atherosclerotic Heart Disease (ASHD), diabetes, hypertension, smoking, obesity, and/or oxidative stress/aging, endothelial dysfunction is often present, due to chronic inflammation and oxidative stress. This increases the risk of clots. Eat foods containing antioxidants (dark chocolate without heavy metals, berries, spinach, kale) for natural sources.

  • Omega 3 FAs 1-2 grams/day (OrthoOmega, Orthomolecular)
  • L-Arginine: doses range from 1.5 grams/day, to 2-3 grams TID for improved athletic performance
  • Extra folic acid with vitamin E, using supplements like Arterosil HP (Designs for Health) BID. This contains MonitumRS™, a proprietary extract of rhamnan sulfate, one of the best studied endothelial glycocalyx-regenerating compounds, from the green seaweed Monostroma nitidum.

Level 7: Pulmonary Support

Add to Level 1 and 3 Support, Plus Level 10 Support (If Frequent Infections)
Avoid cigarette and wildfire smoke (check local air quality alerts), check for indoor mold/radon exposure, use indoor HEPA air purifiers, stay well hydrated, minimize dairy (mucus producing), and stay up to date with essential vaccinations (pneumovax, flu, RSV, Covid).

  • Higher doses of magnesium 750 mg/+ QD and glutathione 1000-2000 mg QD, BID act as natural bronchodilators. I use higher doses of GSH during acute Covid up to 2000 mg TID and other viral infections like the flu, since many viruses need to lower GSH to replicate, and GSH has anti-inflammatory properties. See the link on our website to our initial publications we published on Covid, and doses we have recommended to our patients for prevention as well as active infection: COVID-Prevention-Treatment-Protocols-Dec-2020.pdf These have worked extremely well in our clinic for our patient population.
  • NAC 600 BID, up to 1200 mg BID; acts as a natural mucolytic, as does Bromelain, 400 mg BID (empty stomach). Watch for interactions with blood thinners.
  • Higher doses of omega 3 FAs 2 grams QD also support lung function, as does Vitamin E 300 IU QD.

Level 8: Vision Support

Add to Level 1 Support
Vitamin A
(carrots, sweet potatoes, spinach) for night blindness, antioxidants with vitamin C (bilberry, blueberries, citrus, leafy greens) and vitamin E (nuts, seeds, vegetable oils). Add a vision multivitamin with lutein and zeaxanthin (I-Sight, Xymogen, 2 QD). These compounds are also found in leafy green vegetables, egg yolk, and some fruits for macular support if there are signs of dry macular degeneration.

Level 9: Joint/Tendon/Pain Support

Add to Level 1 Support

Pharmaceutical grade chondroitin 800 mg QD and glucosamine 1500 mg QD together may help to reduce joint pain and stiffness with osteoarthritis, as they are building blocks of cartilage. MSM (methylsulfonylmethane) between 1500-1600 mg/d in divided doses may also reduce joint pain and inflammation. Start with lower doses and slowly increase for tolerance.
We carry two different types of joint and tendon support from Xymogen, called SynovX performance 2 BID x 1 month then 2 QD for joint support; and SynovX tendon and ligament formula, two BID x 1 month, then 2 a day for tendon and ligament support. See the write-up on our website for details.
-Omega 3 FAs 1-2 grams/d, SPMs 1000 mg BID.

Level 10: Enhanced Immune Support

Add to Level 1 Support

  • Transfer factors (3 TF Multi-immune, Researched Nutritionals)
  • Mushroom derivatives 2 QD (MycoActive, OrthoMolecular) with six mushroom extracts (reishi, shiitake, chaga, cordyceps, pearl oyster, turkey tail) which act as biological response modifiers. Can also increase 3,6 Beta glucan (Immunotix) in level 1 support to 1000 mg/day.

If Autoimmunity Is Present

Apart from level 1 and level 2 support, use nutraceuticals that block NLRP3 inflammasomes, and other inflammatory pathways such as JAK-STAT, MAPK, and GAS-STING pathways (these are inflammatory pathways described in detail in our new book, ‘Why We Stay Sick’) and mast cell activation (level 4), while addressing all MSIDS abnormalities associated with autoimmunity (infections, toxins, microbiome issues, leaky gut, MCAS, etc.). Follow an autoimmune diet. See chapter 12 in ‘Why We Stay Sick’ on autoimmune diseases for more information. Do not use enhanced immune support unless specifically indicated.

Level 11: Cancer Support

Add to Level 1, 2 (Detoxification) Plus Level 10 Immune Support

To raise NK cells, in addition to eating an organic diet, high in fruits and vegetables.

Consider Mitochondrial Support, post Chemo/Radiation to block NFKappaB, MAPK, JAK-STAT while stimulating Nrf2 Pathways involved in cancer.
Use other nutritional support based on clinical symptoms/organ involvement/pathways involved in the specific cancer. Level 12: Enhanced Cognitive Support

Increase Brain-Derived Neurotrophic Factor (BDNF) with Level 1 supplements, caffeine, exercise, + Level 3 supplements

Add to Level 1 + 3 Support

  • Gingko biloba 120-240 mg/d, divided doses, evaluate drug interactions with blood thinners, anti-platelet drugs
  • Huperzine A, 50-200 mcg/d, divided doses
  • Vinpocetine 5-30 mg/d, divided doses
  • Membrin (Orthomolecular) up to 1 BID, contains the above supplements.
  • Magnesium malate, glycinate, and especially Magnesium L-threonate (1-2 grams/d, divided doses), watch for GI upset, loose stools and interactions with medications (antibiotics, diuretics). NutraMedix’s magnesium malate and Xymogen’s MinRx (contains mag glycinate) are the primary forms of magnesium I take. Xymogen’s OptiMag Neuro one scoop a day contains magnesium L-threonate, which is the only form of magnesium proven in animal studies to cross the blood-brain barrier.
  • Cordyceps (3-6 grams/d)
  • NADH (5-20 mg/d). Enada brand.
  • Phosphatidylcholine (PC) 1-3 grams/d (Orthomolecular BioPC Pro, one scoop or less per day; vs Xymogen’s liquid Phosphaline 4:1 one tsp/d)
  • Phosphatidylserine (PS)100-500 mg/d (usually taken at bedtime for balancing overstimulated adrenals). Orthomolecular’s AdreneVive is our source for PS, usually taken at a dose of 2 at bedtime in those with high cortisol levels at night interfering with sleep.
  • Vitamin B12 1-2000 mcg/d (dietary sources include meat, poultry, fish, eggs, dairy). We usually use Methyl protect one a day (Xymogen) which also contains methyl B6 and methyl folate
  • Folic acid 400 mcg/d or higher. Methyl protect (Xymogen) one a day, or pure methyl folate (Xymogen’s Xaquil XR) contains 15 mg of L-methyl folate. This dose (and higher) is used during our double dose and high dose dapsone protocols to support red blood cell production.
  • Low dose melatonin 1 mg or less HS (we carry 3 mg tablets and 5 mg tablets through Xymogen, and encourage our patients to cut them in quarters to get the proper dose; the bottle also lasts longer…)
  • Berberine 500-1500 mg/d, divided doses, to block NLRP3 inflammasomes. We use CM Core (Orthomolecular) 3 a day.
  • EGCG, 240 to 320 mg, equal to about 3 cups of green tea/d (always drink organic tea, and teabags without plastic!)
  • Benfotiamine, up to 300 mg BID (Xymogen, Benfotiamine)

Level 13: Sleep Support/Mood Support

Aim for 7-8 hours of good quality sleep per night, to help lower cardiovascular and neurological risks and support a healthy inflammatory response. Getting to the source of insomnia, however, is important so please review our sleep chapter in Why We Stay Sick once it is available. Insomnia can be due to infections (Lyme, Bartonella), sleep apnea, too much caffeine late in the day, stress, depression and anxiety, hormonal dysregulation, prostate issues, etc. so please make sure you have evaluated why you are not sleeping with your healthcare provider. The supplements below have been helpful in our practice as sleep aids.

Add to level 1 support:

  • Copa Calm (OrthoMolecular): liquid GABA/L-theanine 1-2 dropperfuls at bed, and if needed, upon early awakening. This is one of my favorites and can be mixed with other nutraceuticals listed below. It helps calm you down, so it is useful for both anxiety and sleep.
  • SynovX Calm (Xymogen): Valerian root has been validated in hundreds of scientific studies to help with sleep and anxiety. One to two capsules at bed as directed, and as needed during the day for stress/anxiety.
  • Melatonin (Xymogen): I prefer lower dosing of melatonin at bedtime, and this 3 mg tablet can be cut in half if you are sensitive to the effects of melatonin. Levels naturally drop as we get older, so some individuals do need melatonin support, mixed with other herbs to get a good night sleep. I also like melatonin, because low dose melatonin helps block an inflammatory pathway (NLRP3 inflammasomes) involved in certain infections.
  • Cerenity PM (Orthomolecular): two capsules at bed as needed for sleep. I have personally used this with Copa-Calm, SynovX calm and melatonin on nights where I know ‘too much thinking’ is interfering with a good nights sleep.
  • OptiMag Neuro (Xymogen): Magnesium-L-threonate is a particularly absorbable form of magnesium that has excellent penetration into the brain. One scoop at bedtime is the recommended dose for sleep and also for mood and memory support. Click on the link on our site for more information. When mixing this with other forms of magnesium (MinRx, magnesium malate) just make sure you are not taking too much magnesium (you could get loose stools, although some of my patients like it as an add-on for constipation).
  • AdreneVive (Orthomolecular): two capsules at bedtime as needed. I use this in patients who have high cortisol levels on a DHEA/cortisol test, where they are wired at night and need phosphatidyl serine to help support healthy cortisol levels.
  • Herbsom (TCM, Dr Zhang): this contains a mix of herbs including the primary ingredient, 300 mg of Ziziphus spinosa extract per serving. Ziziphus spinosa is a traditional Chinese herb also known as Suan Zao Ren which helps relax the body at night. Dose is 1-2 capsules typically 30 min before bedtime. I have used this in combination with other herbs in patients to help support sleep when I wanted to avoid polypharmacy with medication and potential interactions.
  • Pure Honokiol (Clinical Synergy): This bioactive molecule targets multiple cell signaling pathways critical for optimal function. In the brain, it targets the neurotransmitter receptor GABAA to help reduce anxiety and promote relaxation, positive mood and restful sleep, without dependency or next-day drowsiness. Take 1 - 2 capsules, 2 times per day, preferably with food, or as recommended by your healthcare practitioner.

 

Level 14: Biofilm/Herbal Support for Lyme and Associated Coinfections

We have extensive experience using herbal therapies for Lyme and associated co-infections (including Candida which is a biofilm infection).
