All of our providers are able to diagnose, prescribe, and treat patients. John Fallon has been with Dr. Horowitz for over a decade and follows his treatment protocols. All providers may consult with you at some point during your treatment plan, and Dr. Horowitz oversees the care of every patient. Initial consults with John Fallon include meeting with Dr. Horowitz, provided that the he is in the office that day.
 
Initial consults run for a minimum of two hours, during which the provider will review the patient’s chief complaints, medical history, family history, review of systems and perform a history and physical, order lab tests, and create an assessment and plan. We recommend that patients bring copies of their medical records. There is a great deal of information that will be exchanged during these consults; it is prudent to bring a family member or trusted friend and/or bring a voice recorder. This will help to clarify your treatment protocol and aid with any questions you may have.
 
The office does not currently accept any form of insurance. While certain lab tests and medications may be covered, we do not submit to insurance for our office visits. We will provide the patient with a superbill for you to submit to your health plan.
