After 41 years of diagnosing and treating over 13,000 chronically ill Lyme and tick-borne disease patients, Dr. Horowitz has transitioned from one-on-one clinical care to a consultation-only model. In this model, he no longer sees patients individually in his office. Instead, he works directly with healthcare providers to assist you and your clinician with the diagnosis and treatment of Lyme and associated complex illnesses, allowing continued access to his expertise.

Accessing the Consultation Service

To use this service, you must have a healthcare provider who is willing to work collaboratively with Dr. Horowitz. This service is not available to patients without a provider.

If your provider is interested in a consultation or in learning more about Dr. H’s effective, established protocols, please contact us at medical@hvhac.com. We are working on setting up an automatic scheduler. In the meantime, please provide potential dates and times, and we will get back to you with availability.

 

Training Course for Healthcare Providers

Dr. H also offers comprehensive training videos for healthcare providers to enhance their understanding of Lyme and tick-borne diseases. This course includes occasional opportunities to ask Dr. H questions and helps providers understand the diagnostic and treatment protocols he recommends.

Important Consultation Details

Consultations are conducted with the healthcare practitioner present on the call.

 

Patients and providers may submit documentation in advance, including:

• Chief complaints
• History of present illness
• Social, family, and environmental history
• Current medications
• Review of systems
• Recent history and physical examination
• Active complaints/diagnoses and treatment responses

 

Following the 16-point MSIDS map (see link above) is especially helpful for efficient evaluation.
Please only submit documentation after a consultation time has been scheduled.
Do not send extensive emails or records beforehand.

 

Patients may submit a brief video testimony or written case overview, but the consultation itself is strictly provider-to-provider to support your care.

If you have been a patient of the Hudson Valley Healing Arts Center, all medical records will be securely transferred to Morgan Records Management, LLC (“The Records Custodian”). Morgan Records Management will maintain your records in accordance with all applicable confidentiality and security standards.
